Organs are given to individuals based on their proximity to the hospital, how long they have been on the waiting list, how sick they are, and size comparison, among others. Patients are excluded if they live too far away, if they have a bacterial infection, and if they are too small for the organs to fit. Therefore, the 4-year-old patient is too small for the lungs and the patient with bacteremia cannot get the lungs due to the risk associated with the immunosuppressants. The individual who lives 4 hours away might be able to get the lungs, but he has been on the waiting list for less time than the 18-year-old who only lives 60 minutes away. The 18-year-old with CF who has been on the list for 2 years would get the lungs.
